Description

The Surveys addon adds specialized survey fields to Formidable Forms, including Net Promoter Score (0-10 scale), Likert scale, text or image ranking, and clickable button versions for radio, checkbox, and scale. Marketing and product teams build complete questionnaires with NPS, collect responses within the builder’s standard flow, and view the data in dedicated panels for each field type, turning the plugin into a structured research tool within WordPress.

Key Features

  • NPS Field 0-10
Adds a Net Promoter Score scale with numbered buttons from zero to ten in the form.
  • Likert Scale
Presents multiple statements with the same range of options in a single field.
  • Image Ranking
Allows ordering alternatives displayed as text or draggable visual cards.
  • Radio and Checkbox as Buttons
Converts choice fields into clickable buttons, optionally with associated images.
  • Results Panel
Displays visual reports specific to each field type in the WordPress admin.

Benefits of Surveys

  • Continuous satisfaction measurement
Runs recurring NPS surveys without needing an external feedback platform.
  • Higher response rate
Buttons and images replace long lists and reduce form abandonment.
  • Analysis without exporting
Reports appear ready in the dashboard, eliminating the need to download CSVs for spreadsheets.
  • Leverages the builder
Surveys use the same Formidable Forms editor, without learning a new tool.

Who Is It For?

  • Marketing teams that measure NPS, CSAT, and satisfaction periodically.
  • Product teams that conduct usability studies and feature validation surveys.
  • Academic and professional researchers who administer questionnaires with Likert scales.

For those already using Formidable Forms as a data collection foundation, the Surveys module completes the cycle with dedicated survey fields and reports separated by question type. The combination of NPS, Likert, and visual ranking covers everything from quick satisfaction pulses to long-term behavior studies, keeping all responses within WordPress and accessible in the same submissions panel of the core plugin.
